We are seeking Physical Therapist (PT) to take care of our patients at their homes

Essential Responsibilities
 

• Thorough understanding of Physical Therapy treatment modalities and equipment. 

• Demonstrated ability in functional, cognitive, and environmental-assessment skills. 

• Effective leadership qualities, professional judgment and development. 

• Desire to work in collaboration with clinicians to provide a quality patient program. 

• Basic understanding of anatomy, physiology, mental and social needs of patients. 

• Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served at the patient’s home.

• Knowledgeable of the principles of growth and development. 

• Able to assess data reflective of the patient's status. 

• Interprets appropriate information necessary to identify each patient's age specific requirements and special needs.

• Think critically to mitigate complex problems and make decisive judgments with minimal supervision.

• Work in a demanding environment, balance multiple priorities and take appropriate actions.

• Document the patient’s chart in the EPIC-compatible and HIPAA-regulated MatrixCare Electronic Medical Record System.

 

Requirements
 

• Current State of California Physical Therapist License

• Graduate of an accredited physical therapy program

• Current American Heart Association BLS-HCP
